MARSI's Unique Query Process


Queries are a very specific communication to the physicians, N.P’s, and P.A.’s that identify and request clarification of all documentation — or inadequately documented information — found in the patient record during the review process. We educate the physicians to the process, and then we provide very specific queries for physician input. Queries are generated by MARSI for overcodes, undercodes and missing components. 

The request for clarification, "the Query," references the patient, the DOS that contains the documentation that needs clarification, and the specific entry that needs clarification.  Missing components can be addressed at any time.

Overcode and undercode queries are sent to each physician’s office where they are placed on the patient’s chart. 

During the next visit, the query should be addressed by the physician. The queries are self-explanatory and educational.

MARSI provides a physician help-line and on-going training for immediate response to questions. 

Once the patient has been seen and the documentation issue has been addressed, a copy of that progress note and code assignment should be sent to MARSI. We review the chart to determine if the discrepancy was addressed, documented, and coded defensibly.
